Website privacy policy

What we collect

We only collect information you choose to send us. The contact form collects your name, email address, optional phone number, the reason for your inquiry and your message. The refill request form collects the patient name, date of birth, phone number, optional email address, the prescription numbers or medication names you list, your fulfillment preference and any notes you add.

We do not ask for, and you should not send us, detailed diagnosis or medical history through these forms.

How that information is used

Form submissions are delivered by email to pharmacy staff so we can respond to your inquiry or process your refill request. We use the information for that purpose only. We do not sell it, rent it, or share it with third parties for marketing.

Security, and its limits

This website is served over HTTPS, so information is encrypted in transit between your browser and our server. However, email is not an encrypted channel end to end. That is why our forms ask you to confirm you understand this, and why we ask you to keep clinical detail out of them.

For anything confidential, please use the secure patient portal or call the pharmacy on (281) 252-4200. The portal is operated by our pharmacy management system provider and is subject to its own privacy and security terms.

Cookies and analytics

This website does not set advertising or tracking cookies, and does not build a profile of you. If we add website analytics in future, this policy will be updated to say so and what it collects.

Embedded content

Our contact and home pages embed a Google Map so you can find us. When that map loads, Google may receive your IP address and set its own cookies, under Google's privacy policy rather than ours. Links to the patient portal and to appointment scheduling take you to third-party services with their own policies.

How long we keep it

Inquiry emails are retained in pharmacy email for as long as needed to handle your request and to keep an ordinary business record. Information that becomes part of your pharmacy record is retained under the record-keeping rules that apply to pharmacies in Texas.

Your choices

You can ask us what website inquiry information we hold about you, and ask us to delete it, by calling the pharmacy or emailing show@triumphpharmacy.com. Requests about your pharmacy or health record are handled under our HIPAA Notice of Privacy Practices — ask any staff member for a copy.

Children

This website is intended for adults. We do not knowingly collect information from children through it. Parents and carers may of course submit a refill request on behalf of a child in their care.

Changes to this policy

We may update this policy as the website changes. The current version always appears on this page.
